Return completed form to HRS by November 30, 2009. Some dependents (e.g. spouse, student) may require additional certification.

Enroll or re-enroll in a Flexible Spending Account (FSA)

Return completed form to HRS by November 30, 2009.

Enroll or re-enroll in the Dependent Care Assistance Program (DCAP)

Paper Form Required

Return completed form to HRS by November 30, 2009.

Costs for Medical Benefits:

All medical plans will cover the same benefits as in 2009, but some plans will add or increase deductibles as well as increase other member out-of-pocket costs in 2010. Take a look at the approved changes for 2010.

2009 Medical Plan Premiums

Flexible Spending Account & Dependent Care Assistance Program:
A Flexible Spending Account (FSA) allows you to set up a tax-free savings account to help pay for your and your family’s health care costs. The Dependent Care Assistance Program (DCAP) allows you to set aside a pre-tax portion of your earnings to use for eligible dependent care expenses. Visit www.pebb.asiflex.com for additional information on FSA and DCAP. A paper form is required for enrollment, please do not enroll online. Return completed enrollment form to HRS by November 30, 2009.

Already Enrolled in an FSA or DCAP?
Don’t forget, you must complete a paper enrollment form during open enrollment to continue your FSA or DCAP participation for next year, even if you don’t wish to change your contribution amount(s).

Uniform Medical Plan Updates

HCA has determined that United Medical Resources (UMR) will continue to provide administrative services for the Uniform Medical Plan through 2010 as the interim Third Party Administrator (TPA). UMR has provided TPA service to UMP for a number of years. As a result, no signigicant changes are expected for UMP in 2010.
