Benefit Changes

Use the forms below to update your benefits when you've had a change in dependent/family status including marriage, adoption, newborn, divorce, and other changes.

Update your Medical and Dental Plans

Medical and Dental Enrollment & Change Form:  Use this form to enroll newly acquired dependents (e.g. new marriage/domestic partnership, newborn, adoption) within 60 days of becoming eligible.  This form is also used update name and address changes, and to terminate coverage for dependents who are no longer eligible.

Note: The initial three weeks of coverage for newborns is automatic and reported to health carriers by the hospital.  However, employees still have 60 days to enroll a newborn permanently.

Spouse or Qualified Domestic Partner Certification:  This form must be completed If enrolling a spouse or qualified domestic partner.

Update your Life Insurance

Life Insurance Enrollment & Change Form:  Use this form to request additional coverage for yourself or a new family member.

Beneficiary Designation:  Use this form to update your beneficiary designations.

Update your Retirement

TIAA-CREF Beneficiary Form:  Update your TIAA-CREF retirement beneficiaries online or access the paper form.

TIAA-CREF Name Change Form:  Use this form to update your name with TIAA-CREF.

PERS Beneficiary Form:  Use this form to update your PERS beneficiary designations.

PERS Name & Address Change Form:  To be completed by inactive members, retirees and beneficiaries receiving benefits.  Active members should update their address through their myWSU account, www.my.wsu.edu, and should update their name by using the link below.

WSU Name Changes:  Official name changes with Washington State University.
